Was the sample from a blood gas syringe ?
"electrolyte balanced" fluid in a blood gas syringe contains 9-12 mmol/l
calcium, and should mostly be expelled prior to sampling. If fluid not
expelled, and sample is small - QED

Dear All
We recently have seen an apparent interference in a particular
patient with chronic renal failure due to SLE. The total calcium,
measured on a Roche Integra, of greater than 4 mmol/L attracted our
attention. The method uses o-cresolphthalein complexone. The patient had
no symptoms of hypercalcaemia. On repeating the assay, in dilution, a
lower result was obtained. On further checking, neat, the result had
fallen even further. If anyone has had any experience or knowledge of
this, any information would be appreciated.
