Telepath has a useful specimen notepad for comments regarding advice on
particular specimens or patient notepad for comments regarding patient
treatment etc. I, and my colleagues, put comments in the relevant
notepad and train our juniors to do the same.
At home I keep a simple record of actions taken and calls taken and
advice given. Its on a clipboard near the phone and can be completed
during the call or takes 30 seconds afterwards.
The computer record has helped in dealing with complaints where we were
peripherally involved BUT I have never had to produce it (yet) in a
court of law so I have no idea how fireproof it is. However, even this
basic record has proved its usefulness in risk management in dealing
with questions at Trust level.
As for my home record - its probably no better than medical notes -
subject to handwriting query! But having a record of any sort is
usually a good start in dealing with queries.
