Hsp-70 is not the only protein against which antibodies react in autoimmune
inner ear disease. Probably only about a quarter of the patients have
anti-hsp-70 antibodies, and the antibodies also occur, although with a
lesser incidence, in normal individuals. If you run the patients' samples
against whole inner-ear preparations, you'd find patterns of reactivity
against clusters of antigens of around 90, 40-50 and 15-20 kDa mass, and
these appear to vary greatly from patient to patient. Therefore, an assay
that checks the presence of antibodies against preparations of
whole-inner-ear antigens, rather than specific proteins such as hsp-70,
would probably offer a much better sensitivity. The question with these
tests is what to do if antibodies are found in a patient. Also, given that
the test is not completely specific, can the presence of antibodies in an
affected individual be automatically linked to the condition? And
importantly, would the test result affect the management of the patient?!
If you decide to go ahead with the test against inner ear antigens, the
