This raises several points. First, higher concentrations tend to be needed
to achieve control in acute illness than are required for prophylaxis. And
plasma concentrations are only a guide to treatment: slavishly to tailor
the dose to ensure that plasma concentration is within preset limits
without reference to the patient isn't good medicine.
