We never take patient's blood without both a requisition, which may be just
be a piece of the physician'ss note paper, and a requesting doctor's address
or LIS code. Thereis nothing worse than having an abnormal result and it
having nowhere to go. It is also ilegal to do a test without a reqesting
doctor in Quebec. The one exception is blood which comes from a birthing
centre which may be requested by a midwife. Even this has its problems as
they also do not want to deal with the problems of HIV follow etc.

I would be interested to discover the policy that other labs have when
patients arrive at the phlebotomy clinic without a request form? In
particular, if the patient's doctor cannot be contacted is a request form
completed and blood taken based on historical requesting patterns and
discussion with the patient? If so is patient consent documented in these
circumstances?
