Don't be nervous: most GPs think most laboratories are great. I suggest
you write a short questionnaire first and try it on a few GPs: you'll
probably find you change the wording of the questionnaire in response.
General questionnaire advice:
* Keep it short, preferably less than one side of A4. Most GPs receive
vast amounts of bumf.
* Promise to give the respondents the results, then do
* Put on the questionnaire (not on a separate sheet) where to send it
back to
* For check box answers on scaled question use an even number of options
I think the best questions are:
* Tell us something we do well...
* Suggest an improvement...
