Dear All
We have a case who had a dramatic cardiogenic
presentation in whom myocarditis is highly suspected
clinically. Serial TnI, TnT and CKMB mass on three
samples 12 hours apart are all negative. The subject
of using troponins in suspected cases of myocarditis
is not thoroughly covered by the scientific
literature. It seems that elevation of the new cardiac
marker is only observed in 34% of cases of confirmed
myocarditis with very little information on what
levels of TnI or TnT you would expect to see in such
cases (TnT 0.59+/-1.68 ug/L in one reference, not a
very high rise surprisingly enough!!).
I would welcome any real life experience of list
members of using troponins in cases of myocarditis.
What sort of levels and temporal release pattern of
troponins you encountered in such cases?
