Hi all
We had an incident where two skin biopsies went missing somewhere between
the out patient clinic in a satellite hospital and the lab, a journey
which involves a van run. Although this involved samples for Histopathology
we have been asked to review protocols for all samples from the point of
view of security. Our view at this point is that samples which are of
critical importance , unrepeatable in this case, should be treated like
emergency bloods ie the lab informed that they are coming. At least then if
expected samples do not arrive investigation can take place at the
earliest.
Has anyone had to address a similar problem or does anyone go so far as
tracking samples in some way? I would be grateful for any ideas.
