I was more interested in the efficacy of the test in screening out (in
general practice) prior to referral.

Don't assume that specialists will use these tests less. There is good
evidence to show that heart failure treatment is more effective when
a) specialist cardiologists rather than generalists are involved and b)
they use more diagnostic tests (see JACC 1997;30:733-8)
