Dear Colleagues
We have been dealing with a FT4 precision and "flyer" problem for
the past six months on the Immulite 2000. We have run all FT4 samples in
duplicate over this period and can confirm the figures quoted by the other
contributors on this subject. DPC(UK) accept that there are performance
problems with the assay at our site. However this has not been acknowledged
(to our knowledge) by the parent company in the States. The bead coating
process has been updated a number of times, and we were assured that from
Kit Lot 204 there would be a marked improvement in performance. We are
currently evaluating this Kit and have indeed seen encouraging results with
regard to precision but have still observed a flyer incidence of about
1:250.

At this moment we are evaluating the quality of the FT4 on our
Immulite 2000 by running 10 patients per day in duplo. This
because we saw a flyer (duplo of 25.8 and 12.8) and had the feeling
that there were more. This feeling is a result of comments from our
doctors concerning unexpected FT4 results.
It turns out that on one day about a thirth of the duplo's had a
difference between 10 and 20% of the average, whereas on other
days all duplo's have a difference below 10%. We are still looking
at it but are at this moment considering an analyser problem as the
cause and not the 'chemical' characterisitics of the test.
