American Academy of family practice has defined practice policy and goes on
to say when it becomes a guideline.
Practice Policies -- Practice policies are recommendations issued for the
purpose of influencing decisions about health interventions.
Guideline -- A practice policy is considered a guideline if the health and
economic outcomes of the interventions are sufficiently well known to permit
meaningful decisions and an appreciable but not unanimous majority agree
which intervention is preferred.

In my limited search, I found that the terms protocol and guideline are
being used as synonyms by many organisations.
