Medecine has it's cultural differences
One of them is that corticosteroid is given early here
So it can act early
Seen it, read it, done it.
Methylpredisolone & Dexamethasone have a peek effect at 1-2 hours
Kids get a Dexamethasone syrup
Adults get Methylprednisolone IV (unless things have changed)
==> Charles - I also agree about the importance of giving steroids early,
despite the very valid points raised in argument.
1. Some Steroids work earlier than the measurements mentioned on the list.
2. Many of those which reach measurable effect at X-time have a milder
effect earlier and we are not yet 100% experts at this "measuring".
3. The effects on asthma symptoms and signs experienced at the time is NOT
the main reason for giving steroids - it is the prevention of aggravation
and progression.
4. Still seeing significant numbers leaving department and heading to wards
without having had their 1st dose and delays which follow may prolong
admission and/or put patient at risk.
5. Any harm in early administration?
