Dear Peter,
The Oswestry disability questionnaire is most often used, though the Rowland
Morris questionnaire has recently been reported to be more accurate in
reflecting changes in the low back. The Neck Disability Index is useful for
the cervical spine. The literature on both can be found in Spine and APTA's
JOSPT.
