According to "The Church Visible" by James-Charles Noonan Jr. (Viking,
1996), p. 379 under "A History of Papal Titles":
Leo the Great added the title Vicar of Peter, which was soon altered to the
more theologically correct Vicar of Christ on Earth."
Pope St.Leo the Great (September 29, 440-November 10, 461). Though the book
doesn't get more specific about when the 'Vicar of Christ on Earth' title
was altered, it does give a very detailed account of the history of papal
titles, which I would be happy to scan in for you if you'd like, just

This book, The Church Visible, details the ceremonial life and protocol of
the Roman Catholic Church, and has everything you could ever want to know
about church protocol. It is INCREDIBLY interesting, with lots of color
pictures of papal and cardinal heraldry. I'm sure you can get it at
Amazon.com or B&N.com for about $35, but it is well well worth it.
It also tells you where to seat the pope and various heads of state and
cardinals when you throw a dinner party for them (so as not to commit an
embarrassing papal faux paus), as well as how to address their invitations.
It also includes a sample menu from a papal dinner.
