I used to, my boss still does. No evidence, but never had a bier's fail
with or without extra torniquet round distal forearm.

Occasionally, but consider it 2nd best: significant failure rate, but
then only using 7-10mls 1% lignocaine, which was what I was always
taught was the appropriate volume. Maybe I need another 15 years
experience too!
