I haven't got copies to email to you, but I have got some info which might
help your colleague obtain the measures. 

If you get replies with the measures electronically, or anyone else is able
to send them by email, I would be delighted to if you could send them to me
too.

Shoulder Pain and Disability Index. 

SUMMARY: Patient-completed index consisting of 13 items divided into pain
and disability subscales. A visual analogue scale is used for each item,
which is translated into a numerical score. A calculation is then required
to obtain an overall score. Scores range from 0-100, the higher scores
indicating greater impairment. TIME TAKEN (MINS): 5. TRAINING REQUIRED:
None. EQUIPMENT REQUIRED: None. COST: Not given, questionnaire reproduced in

Shoulder Rating Questionnaire

SUMMARY: Patient-completed questionnaire to assess the severity of symptoms
and functional status of the shoulder. Consists of six domains (global
assessment, pain, daily activities, recreational and athletic activities,
work and satisfaction), each graded separately and weighted to produce total
score. TIME TAKEN TO COMPLETE (MINS): 5 to 10. TRAINING REQUIRED: None.
EQUIPMENT REQUIRED: None. COST: Not given. Copy of questionnaire in
L'Insalata (1997). PATIENT POPULATION: Disorders of the shoulder. LOCALITY
FOR USE: Suitable for community, inpatients, outpatients and rehabilitation.

Shoulder Disability Questionnaire (SDQ) 

SUMMARY: Patient-completed questionnaire covering sixteen physical movements
or activities of the shoulder over a twenty-four hour period. Scored on a
dichotomous scale, positively scored items are divided by the total number
of applicable items and expressed as a percentage, 0% being no disability
and 100% being very disabled. TIME TO COMPLETE (MINS): 5. TRAINING REQUIRED:
None. COST: Nil. Questionnaire reproduced in van der Windt (1998). PATIENT
POPULATION: Those with shoulder pathology. There may be other populations
involved in other studies. LOCALITY OF USE: Community, inpatients,

Western Ontario Shoulder Instability Index (WOSI) 

SUMMARY: Patient-completed quality of life, which can be used by any
clinician or by mail. Comprises 21 items representing 4 domains (physical
symptoms, sport/recreation/work, lifestyle, emotions), each scored on visual
analogue scale. TIME TO COMPLETE (MINS): Not specified, estimate 10.
TRAINING REQUIRED: None indicated. EQUIPMENT REQUIRED: None. Copy of
measure, including instructions to subjects, explanation appendix and
scoring instructions available from Dr A. Kirkley, Fowler Kennedy Sport
Medicine Clinic, 3M Centre, The University of Western Ontario, London,
Ontario, Canada N6A 3K7. COST: Not specified. PATIENT POPULATION: Patients
with shoulder instability. Designed as primary outcome measure in clinical
trials evaluating treatments for shoulder instability. Also suitable to
monitor progress in clinical practice. LOCALITY FOR USE: Not specified.

domains: A 4 point pain scale (none - severe) measuring pain experienced at
rest, in motion, at night, during sleep and when lying on affected side. A 4
point scale (none - past the elbow) recording the degree of radiation and
thirdly the amount of pain on a 0-100 scale. TIME TO COMPLETE (MINS): 5.

Upper Extremity Functional Scale (UEFS).

and washing dishes, scored on a 10 point scale. TIME TO COMPLETE (MINS):
