I have just spoken to Ben at NOF who says if you have already started
typing/word processing the form in upper/lower case then you don't need to
change it - but if you're starting from scratch put it in upper case !!!!!
Perhaps Earl could ask someone at NOF to put a definitive answer on these
lists before we all go insane!
