Hi Wende,
I have no personal experience of intra umbilical syntocinon, but having
reviewed the literature thought it was of little benefit. I would be very
interested in receiving a list of references from you. Have you looked at
any of the following?
Carroli G etal (1998) Intra umbilical vein injection and retained placenta
Brit J of O and G Feb 105(2) 179-85
Bider et al (1992) Effects of intra umbilical prostaglandin F2 alpha
injection on the 3rd stage of labour J of Reprod Med 37:317-319

I am behind with my email reading as evidenced by my
response to a post
dated May 16 re: intraumbilical syntocinon.
This has been used with success at least 4 times that I am
aware of at
my workplace in the last four years.
After reading about it in a US journal I raised the subject
at a
perinatal meeting. One of the GP Obs (Dr Conservative) was
intrigued
and asked if I could supply info. As most of it came from
the medical
(not nursing) library at Flinders U I found this vexing. It
would have
been easier for him I should imagine. Anyhow he tried it
out (with me)
one night - and voila, it worked, - as in the
subsequent 2 times I
was on.
A (young) GP tried it out one night also, at my suggestion,
but I had to
reassure him++ that Dr Conservative had accepted the
technique.
My husband (then Night Charge) informed me once that Dr
Conservative had
told him that he was angry with his wife (me!) because the
night before
I had not "reminded" him of the technique. This chap is not
one that
one "reminds" and because he didn't suggest it I assumed
that in this
particular case he didn't want to use it. Damned if you
do, damned if
you don't.
I would suggest that it is a good technique to learn and to
use if the
usual 'tricks' don't work (such as nipple
stimulation/bottle-blowing/panning etc.) and the alternative
is a G.A.
Andrea,
The reason it works is that the synto. is in at least 20ml
& injected
near the introitus. There are various theories as to the
effects of
this, e.g.:
1) the synto. acts locally with the subsequent cont.
shearing the
placenta.
2) the fluid volume at the interface of uterine wall &
placenta shears
the placenta off.
Our (new) CN is currently reading my lit but if wanted I
will post the
refs when I get the articles back.
Wende.
