We have just developed a reply form, to accompany an incomplete
request form back to the ward. Options wich can be marked are:
Incomplete patient identification, information too hieroglyphic, test
has already been done (date), incorrect sample material, signature
of SHO wanted, etc.
Labelling policy can mean a large number of things...

Does anyone know of literature or have personal audits of problems
with specimen labelling and/or the completion of request forms?
