Hi there, I am interested in your experiences of case-loading models of midwifery with a mixed risk group of women (as in social and health risks), in particular the size of the case-load. It seems usual in Australia for employed case-loading midwives to take a case load of about 40 low risk women and I am looking for some information that might offer us some guidance in establishing the case-load for emplyed midwives caring for a mixed risk group? Do you have any advice or publications that you know of that might be useful?

Your request came last week while I was away.
I wonder if you have considered using a quality of life tool along with the other instruments.
The Mother-Generated Index has been validated for postnatal use (Symon et al 2003), and has been used during pregnancy too (other references below). Its value is that it provides insights into what the woman really thinks, so can provide a lot more useful information than that collected using standard tools. It’s complementary in that sense.
The MGI form does need some explanation, because it’s not the usual ‘tick-box’ approach used by so many survey forms. It also requires a certain level of articulacy, but in practice we’ve found that once women see the point of it they’re quite comfortable with expressing their thoughts in this way.
It’s been used in several countries now (see JAN 2012 article cited below).
