Sorry for the mass email. I'm just wondering if anyone in our network has ever had to draft a template agreement for when research subjects (or even non-research subjects) wish to provide them with documents?

My IRB is asking me to create a template agreement for such occasions should they arise... never mind the impracticality of such a thing for research subjects who may wish to provide me with information anonymously... sigh...

Has anyone had to do this? If yes, might you be willing to email me directly and share your experiences/template agreements?
