School experiences in children at high genetic risk of mental health problems.

A number of genomic disorders have been identified that are associated with high risk of childhood psychiatric disorder and learning disability. It remains unclear how these children experience their school environment. This will be studied and the links with cognitive function and psychiatric risk evaluated longitudinally.

Abstract

Objective: Recently, chronic irritability has been considered to have a bi-dimensional nature, composed of tonic and phasic phenotypes. No study has explicitly explored these dimensions in clinical samples. This research aims to ascertain the validity of this structure, in a clinical sample of children with Attention-Deficit/Hyperactivity Disorder (ADHD).

Method: The sample consisted of 580 children with an ADHD diagnosis, aged 6-18. 194 of whom were followed up 2 to 5 years later. Parent-rated semi-structured interviews and questionnaire measures were used to formulate research diagnoses and collect clinical symptoms of ADHD and several associated conditions. Maternal psychiatric symptomatology was collected through self-report questionnaires. Irritability components were assessed extracting items from the Depression and ODD subscales of CAPA.

Results: Overall, both were common; 50.7% had tonic irritability and 18.6% had phasic irritability, whilst 15.3% of children had both. Cross-sectional results questioned the validity of this distinction as both subtypes were significantly associated with most of the clinical correlates considered, with the same strength of association. However, family history and longitudinal analyses revealed a more specific pattern of association between tonic and internalising disorders and between phasic and externalising disorders, with a significant different in their strength.

Conclusion: This study is the first to test the validity of separating tonic and phasic dimensions of irritability in clinical populations. The results question the validity of this approach cross-sectionally, although external validators suggest more specific associations. These findings are important for our understanding of the clinical irritability and the future development of tailored interventions.
