This won't answer all your questions, however, I would suggest not using the Glascher & Gitelman tutorial as it contains a number of flaws for implementing partitioned-error approaches to mixed-measures designs. Have a look at the preprint here (https://psyarxiv.com/a5469/) for details about correctly implementing these types of models in SPM. In short, you'll need 2 models, one for the within-subject and within-subject x between-subjects effects, and one for the between-subjects effect.

Once you've understood what you need to do, perhaps have another look and see which of your questions remains?
