I have been asked the following question:

 

Would you know where I could source information on the hospital or NHS trust with the highest number of medication errors, if possible specifically insulin administration errors and also the trust with the lowest number of errors?

 

I suspect it won’t be, but does anyone know if this information is available anywhere?
