Hi, This is something I'd had concerns about (sharing sometimes very sensitive information with awarding bodies) so created a special consideration request form that students have to complete before I will submit an application on their behalf. Although it doesn't include a specific DPA declaration it means that they decide how much, or how little, information they are willing to share. I ask for medical evidence for any request being submitted on the grounds of ill health and I also ask for a counter signature as I was getting anecdotal tales that students were inventing seriously ill relatives in order to qualify! I've attached my form which you're welcome to use/amend as you wish. You tell students this is going to happen (what data we share, how long etc) in the privacy notice when we first collect data from them (ie Enrolment). You certainly don't need a signature as you're not getting consent. Does this help? We are required to have a Data Protection form signed before we submit any application for an Access Arrangements due to the information we are including but what about special consideration?
