Andy,

the screenshot you attached verifies that something went wrong, but not 
at what point. As I said, go through every step in succession and check 
that afterwards, things are as they should be. As I also said, the most 
likely reason (at first glance) seems to be that the image(s) you 
derived the normalization parameters from and the image(s) you applied 
them to are not in register.
