Multidisciplinary Pathology
Request Form
Could I ask whether anyone has managed to combine the requesting for all pathology disciplines onto a single request form? We have successfully combined haematology and clinical chemistry, but we are now being asked to combine microbiology and histopathology onto a single pathology request form. Our plan is of course ward order comms, but in the meantime has anyone got an example of a single request form for their whole pathology department?
