You might want to take a look at Hiawatha Software's TagGen suite of
metadata creation tools and add-ons
(http://www.hisoftware.com/prodlist.htm). The add-ons are available for a
number of commercial products like FrontPage, Hot Dog and WordPerfect, and
much like you've outlined they incorporate a metadata creation form into the
product's user interface. Though they have a robust stand-alone app for DC
creation (TagGen-Dublin Core), the add-ons are generic metadata creation
forms. The latter, however, are flexible and customizable. I've created a DC
simple form for FP, and I understand that the software developer is working
on a version that will allow the creation of qualified DC forms. The
products also support GILS and WAGILS.
All in all a good product, and reasonably priced.
