i just received a copy of this book yesterday. It is an absolute must for
clinicians. There are new chapters on osteoporosis, HIV, urinary incontinence
prostate screening ,vaginitis etc.The first chapter does mention on diagnostic
reasoning along with details on lab errors and variation and factors other than
disease which affect the lab values. I use these books in a big way to teach
medical students and residents and the usual response is " Why did other people
not tell us before that we could practise medicine like this?"The main reason is
that clinicians rarely use bayesian logic, likelihood ratio to interpret their
testing. In a paper by A Feinstein group at CT ( check out Bandolier for details
on this paper ), it was made clear that clinicians get most worried about false
positive tests ( especially Surgeons), while false negative tests ranked number
2 in the things that people cared about in practice.
