There was an excellent study by Mike Klinkman and Lee Green in the primary
care setting on the causes of episodes of chest pain (initial presentation).
If I recall correctly, it found that the three major causes were panic
attacks/anxiety disorder, GI problems, and musculoskeletal chest pain (about
30% each). If you search PubMed on their names you should find the
reference quite easily.
