add to my original question a secondary one; what of the
'shrines' that are sometimes found at waysides, or on former
pagan sites. Do these have any ecclesiastical validity in
the way that the chapels discussed so far have, or are they
manifestations of popular religion to which no rules apply?

Although I don't have references, I know there are studies on these
roadside shrines in Quebec (which is perhaps the most "medieval" part
of North America!), but as for Europe, one study you might consult
