When I worked at Hughston Clinic in Columbus Georgia, we had an orthopedist
come to work there who had been chief of orthopedics at a large military
hospital, I think Walter Reed. Anyway, they used CPMs where he came from
and had to schedule several days a month for to perform closed
manipulations. After he had been with Hughston Clinic for several months,
he realized he hadn't done any manipulations and everyone was doing fine.
We almost never used CPMs. He realized that the PTs were appropriately
aggressive at getting motion back after surgery. It is not that CPMs are
bad, but they are not a substitute for appropriate stretching post op,
especially with total knees. The research on CPMs does seem to show a
reduction in the use of pain meds. And for arthoscopic surgery where the
capsule of the joint is not too damaged (hopefully), there shouldn't be
much loss of motion anyway. For a total joint or an open procedure, the
CPM will not restore motion--it takes passive stretching by the patient
and/or the PT (depending on how aggressive and compliant the patient is).
