Bart,
In my experience, Botulin injection used to decrease tone for a CVA with
moderate extensor tone in the lowers is effective but only lasts a
couple of months.Injections are done directly to the muscle groups that
are targeted to be overreactive.Repeat injections are done as soon as
the toxin wears out. I have also seen it done in cases wherein there is
muscular imbalance in the ocular muscles mimicking ptosis. A very small
amount of the toxin (around 3cc per site ,3-4 sites in the ocular area)
is injected. I have seen complete resolution of "ptosis" like
manifestation within 4 days after injection and usually lasts 6-8 months
before a repeat injection is necessary. In this scenario, it definitely
is worth it for the patients. There are more references in the
literature relating to specific studies with botox injections for ocular
problems rather than other muscle groups that I have seen. Hope this
