Title: Seamless information: supporting the interface of health and
social care.

The NHS reforms of the last decade, in particular the drive in
health policy towards a primary care-led NHS is bringing into
question the boundary between health and social care. This study day
will draw upon the experiences of information professionals,
academics and practitioners from the relevant fields to firstly
outline the current relationship between health and social care and
to then explore the information needs in an environment where
health care and social care services are converging. The day is
aimed at health information professionals, or more generally at
other health professionals with an interest in gaining a greater
understanding of the interface between health and social care.

An afternoon workshop led by Jo Cooke, Research Fellow, ScHARR and
Lead Research Nurse, Community Health Sheffield, will provide the
opportunity to consider patient care pathways.
The IFM Healthcare Annual General meeting will be held during lunch
