I've been asked to review and update our OH forms.
Regarding our 'new starter' forms (which have replaced 'preemployment' forms), do you think it's generally good practice to ask employees to list all of their medical conditions, disabilities, and medications so that you as the OH practitioner can determine which of them can potentially impact on their work performance and attendance? Or is it better practice to let the employee themselves advise you of the relevant facts in response to a more limited question approach which is designed to put the onus on THEM to tell you their relevant medical and health history?
