What it is IMO depends very much what the 'consent' form says. Unless it is specifically worded, which I doubt, as an authorisation to make a request on his behalf it cannot be a SAR, just a 'consent' to disclose.
My concern would not be the form but, given the source, whether the consent was really freely given and informed and there are clear parallels here with the recent ICO advice to the insurance industry.
Whatever this is clearly poor police practice. Has the subject ad leagl advice ? I would tend to decline the request and suggest they use a proper channel, either a genuine SAR or a proper s29 request.
