I've seen these in my career, usually issued on direct request or when there is no need eg absences of less than 7 days, or when the individual/patient wants to make more of a point than the usual certification would. They attract a charge of between £15 -25
. So there must be a specific reason why this has been requested, and then issued. As with a fit note, it's an advisory note to the individual, up to the employed what they do with it.
