Pucklechurch, I think; in (formerly) south Gloucestershire, near Bristol; geology of lias and Trias capping coal measures. Definitely worth checking 'Jurassic Rocks of Britain' (Fox-Strangways etc) and Arkell 1933 plus the old Survey memoir for the area.
I can't remember it being a specifically ichthyosaurian locality but in an inland site like that, you wouldn't get many ickies unless there were active quarries in the first place.

Subject: help required with an almost illegible label
Dear Colleagues,
I am struggling to read the place name on a label (photo enclosed). Could some
of you take a look please and see if you recognise what is says?
We can only do better than the last person who transcribed this. They read
'jaws of Ichthyosaurus communis Lower Lias' and transcribed it as 'jaws of
Achthiasarus carnivorius, Lower Lias'. They didn't even guess at the place
name.
The only clues are its an ichthyosaur and the sediment isn't typical of the
areas around Lyme Regis or Whitby.
