I received a literature search enquiry this week and struggling to find the right search terms for it.
The healthcare professional who contacted me says the more people responsible for checking something, the less likely it is to be accurately checked (for example, drugs found expired or missing with an audit) - and wonder if there is there is any evidence supporting this principle?
So far, I found a high number of articles on risk and quality management problems (and the failure of organisations to implement continuous improvement to avoid / reduce these problems), but nothing this specific.
