There are some other general HER Access information you may find useful to look at as a bit of background information.
First of all there is the draft Access and Charging Policy template that my colleague Nick made available last week. There are two pages on Access, ahead of the section on charging. The template is available in https://www.jiscmail.ac.uk/cgi-bin/filearea.cgi?LMGT1=HERFORUM&f=/HER_Policy_Templates
There is information in Informing the Future of the Past: Guidelines for Historic Environment Records (IFP) on Access and Charging in Section F.4 - http://archaeologydataservice.ac.uk/ifp/Wiki.jsp?page=SectionF.4 . Although the text was written in 2007 it still contains some useful information.
In addition to these it might be worth looking at the current HER Audit specification to see what HERs are audited on in terms of Access to Services. Whilst it should be noted that there is not necessarily a requirement for HERs to fulfil all of the areas queried in the specification it does perhaps suggest areas for you to consider. Finally there are the current (draft) HER Benchmarks for Good Practice (2002) that you could look at - a copy of which you can find at https://www.jiscmail.ac.uk/cgi-bin/filearea.cgi?LMGT1=HERFORUM&f=/HER_Audit . Section 1 deals with User Services and Access and outlines what is required to meet those Benchmarks. I would recommend using the version that has been cross referenced to IFP2 as this is the version that I have provided the link for above.

Subject: Computer access agreement for HER visitors
Hi
We have been asked by our IT people to get all visitors wanting to access digital HER records and other archaeological datasets to sign our E-policy document. This is actually not an appropriate document for non-staff users as it is 30-odd pages long and covers all aspects of the Authority's activities. I need to outline to them what we require in terms of access but before responding I would like to know if any HERs have had a similar IT experience.
If anyone already uses an agreement/policy document of this type or has any advice or suggestions I would be very grateful to hear about them either on- or off-line.
