Have you tried using the phonetic symbol app for OS 4.0 and above -iPod
touch,  iPad or iPhone from Macmillan called Sounds: The Pronunciation
App. It  has audio output and works with VoiceOver.

  There is a free version with limited access and the full version costs
£3.99

Please be aware that all the menus work with VoiceOver but it does say
'unpronounceable' if it cannot say for example and digraph when you first
tap each symbol with VoiceOver,  but then the audio kicks in with the
correct recorded voice sound.
