Is your input directory writable? At least one of the previous versions of MF seemed to check for write permissions on the input file directory for some obscure reason, even if the output directory was different from the input - and would stop if it wasn't. Not sure if this is the one you are using, this was fixed in later releases. You can check this by copying your raw data file to the output folder and running it from there (ensure it's not write-protected and has sufficient space).
At any rate, v.2.0.21 IS quite old, most users seem to agree that the more recently released 2.2 is much better. I think the most recent one is 2.2.15 which appears to have fixed a number of bugs.

MaxFilter exit codes mean:
EXIT 1: Unrecognized or wrong arguments.
Check the command-line arguments and filenames.
EXIT 2: Failed to open file.
Chek that the input file exists and is readable.
EXIT 3: Input file is corrupted or wrong format.
Check that input datafile is in FIFF format and MEG data is view-
able, e.g. in graph.
EXIT 4: The output file already exists.
Select a new output filename, remove the esiting output file, or try
option -force to overwrite.
EXIT 5: Error in processing a matrix.
Numerical problems, e.g. in inverting an SSS matrix. Check the
head position or SSS origin settings.
EXIT 6: Writing the output file failed.
Writing the output file was terminated. Check that the disk space is
not full.
EXIT 7: Improper SSS parameter encountered.
Check that the SSS parameters are appropriately set.
EXIT 8: Temporal SSS error.
tSSS has found more than 42 waveforms to be projected and cannot
process the data.
EXIT 9: Unknown error.
Other error, please check the error message reported on exit.
Thus, in your case the disk space may have become full?
