Like everyone else so far - we do specifically ask for the Trust's risk assessment next to the box they tick if work related stress. The minimum I accept to take the referral forward is information that they have talked and have plans for a formal stress risk assessment and I get HR on the case if there is nothing at all.
If they haven't yet done one when I see them or they are at home absent already, during the consultation I give them the HSE 'return to work' stress risk assessment. I have found that this is better in some cases that the joint approach initially as the questions are geared to provoke the individual to clarify what the issues are. Then the stress risk discussion with the manager can be more fruitful as it cuts out the non-essentials and gets straight to the point.

If you stop asking, even in the face of continued failure, you could be inadvertently delivering the message that the risk assessment isn't important.
