It is possible that there are issues opening Adobe Reader from Microsoft
Internet Explorer.
I have identified this issue on the Win 7 platform.
There is an update available from Adobe that may fix it.
If the form does not open from source right mouse button it and select "Save
target as" and save it to a folder you select.
A pop up at the bottom will allow you to open it direct, otherwise open the
saved file by either using the "open File" and browse from the reader, or
just double clicking the file.
