Good to hear you allow for uncertainty in your own reporting.
For clarification, what I was trying to say is that without previous report provision, my view is that the uncertainty level in any investigations that rely (to greater or lesser extent) on past reports that are not provided to the LA or appended, can be unacceptable; particularly as many such reports include a caveat that effectively says that results, recommendations or conclusions cannot be relied on outside of the context of the original investigation.

Never really thought about it that way myself, but yes, I can see when done badly (or mechanically) with little regard for what is actually being added in terms of value to the site model, it could be a real pain for a regulator. Will try harder in future Miss.
Two of the main problems when reporting are being accused of bias (why do you think this report is useless?) and omission (why didn't you include/reference this one?). As such some reports and reporting styles I have seen do tend to err a little too far on the side of caution... Of course to an extent you have to play somewhat safe when it is not your report you are commenting on and you often have no strict right to critique it :)

Good point. That may be why we commonly receive reports in which other reports are summarised/referenced but not provided/appended. To be frank, very unhelpful and often introduces an unacceptable level of uncertainty that is over and above that normally associated with inclusion of data that has not been 'self' collected.
