Dear Paul,
Are you correcting for EPI distortions (either using a fieldmap-based approach or a phase-encode-reversed acquisition and reconstruction)? If not, could it be that the problems you note in the frontal areas are due to distortion? This is quite common as you see the distortions most in the frontal areas (more inferiorly) and inferior temporal areas.
