Well, I would say write to the PH whom you are transferring to & tell them what has happened, I don`t think that there is anything else that you can do is there on this one.

I am currently going through the process as you have mentioned & I asked the same question as you have to the transferable HP. I was informed that this very rarely happens but if the individual asks for the notes to be destroyed, shredded or whatever then that is their wish.

Please correct me if I am not right here?

( no one declined so I am ok- but there is always one )

I need to ask a question about transfer of OH records to another provider. 

What happens if an individual objects to the alternatives that have been offered to them re their notes:- 

Transfer to new provider    or 
Store in secure archive of current employer 

and wishes to have their notes shredded or wishes to take them into their own possession? Incidentally, this is not health surveillance  or stuff that needs to be kept by law. 

I am asking this is anticipation of a particular scenario and all comments appreciated......
