There are many things that could potentially go wrong here, and the
behaviour will depend on many of the subtleties of the way you
transformed the data. As a first step, I'd suggest using the Check
reg button to display the transformed tissue probabilities alongside
the data you wish to segment. If the alignment is reasonable, then
the initial affine registration used by the segmentation is more
likely to work.
