Without wishing to be rude or negative, the questions are not well-designed.
I would have hoped for better.
The one that especially leaps out at me is :
5) If so what happened to the remains after excavation? Reburied in
the same location; Reburied in alternative location; Retained for
archaeological research; Stored in a museum; Retained for research at a
University or other Institution
As I see it, 'stored in a museum' is equivalent to 'retained for
archaeological research' and 'retained for research at a university or other
institution' is much the same.
The problem with such questionnaires is that they are often not designed by
people expert in such matters (ie designing questionnaires) and therefore do
not necessarily provide meaningful results. For example, if a unit retains
collections, it is generally on the logic that those remains will be passed
to a museum after publication - admittedly often at some nebulous date in
the future. Sometimes museums lend material to universities for teaching
collections and study. And so on.
