We haven't actually started yet, but we're planning to use iPads (or
even iPhones) plus the PollDaddy app - we're using PollDaddy
<http://polldaddy.com/> online as well, and a single survey can combine
offline and online responses (but is able to distinguish between the
two). Trialling of it indicates it's very easy to use.

A collegue is looking into the possiblty of using a tablet PC for
